Health Forms
Required for all students
Your child's safety is our number one priority! Please review the following information carefully:
- All Health Forms must be submitted by September 5.
- A yearly physical is required, so you will want to make a physician’s appointment as soon as possible.
- If you have questions about Health Forms, contact CarterD@OldfieldsSchool.org.
Please note, a student WILL NOT be allowed to come to campus in September if we do not have all the necessary Health Forms.
